The Chronic Disease Self-Management Program (CDSMP) is a workshop for adults with at least one chronic health condition, which may include epilepsy. It focuses on disease management skills, including decision making, problem-solving, and action planning.

About the event

What is the benefit?

  • Confidence;
  • Physical and psychological well-being;
  • Knowledge of ways to manage chronic conditions; and
  • Motivation to manage challenges associated with chronic disease

How does it work? 

Participants attend a workshop once a week for 6 weeks to learn problem-solving,  decision-making, and other techniques for managing problems common to people with chronic diseases. These workshops are offered virtually on Zoom and facilitated by 2 trained leaders. In a typical workshop,  participants set a realistic goal for the upcoming week and develop and action plan for meeting that goal. They report on their progress at the  following workshop and ask for feedback from the group to help address any challenges. What do I need to participate?  To participate in the program, you will need to register online. We will  provide you with a Living a Healthy Life with Chronic Conditions program  book. You will also need to be available to participate in the 6 weeks of sessions to complete the program. All participants will be asked to  complete surveys before starting the program and then again after completing the program to help evaluate the program.
